Durable Power of Attorney for Healthcare

This Power of Attorney allows someone else to make healthcare decisions for you in the event that you are incompetent at some point in the future. You choose a primary and secondary person to make those decisions.  It is only effective if your doctor and your agent (the person you name) agree that you are incompetent.

Your agent also has 3 powers after your death- 1. the power to authorize an autopsy; 2. the power to donate your body parts or organs, unless you have completed a living will or other document that makes these decisions; and 3. the power to dispose of your remains,
